Diagnosis: Fractured, worn, crooked and discolored front teeth.

Prescribed Treatment: Complete smile reconstruction. Repaired and refilled all anterior teeth using porcelain crowns and bonding. Reshaped existing front teeth and applied porcelain veneers on top front teeth. Adjusted and whitened bottom front teeth.
