Patient: Bill Fitzpatrick
Diagnosis: Severe dental phobia caused patient to avoid dentist, resulting in decayed, discolored, worn, defective teeth, as well as premature tooth loss.
Prescribed Treatment: Tooth colored, composite fillings, porcelain veneers and crowns. Porcelain bridgework to replace upper back teeth, lower removable denture to replace missing lower teeth. Full mouth bleaching & whiting.
